Dentist anesthesiologists Stanley Malamed and Jason Brady told HealthFirst's Health First Talks podcast that fatal local anesthetic overdoses in young dental patients follow a chain of small errors: one-visit treatment plans, plain 3% mepivacaine, four quadrants injected at once and adult cartridges. Oral Health Group estimates LAST at 0.27 cases per 1,000 administrations. Brady caps his anesthetic tray at 90% of the maximum recommended dose.
